A customer consent form is required with all online applications. This document is shorter than the full incentive application, and you can print it and bring it to the customer’s home. The consent form ensures that a customer has read and agreed to the terms and conditions of Energy Trust’s incentive. Make sure you use the appropriate consent form for your application:

• Home Performance with ENERGY STAR^®
• Savings Within Reach
• Manufactured homes

You can scan and upload the signed consent form or take a photo of it with your smartphone. (Make sure the signature is legible in the photo.) Applications submitted online without an attached consent form are considered to be missing information, which will lead to a delay in your customers receiving their incentives.
